Primary Care Physician
We are a busy primary care office looking to fill full-time positions at our practice. We are looking for experienced physicians to add to our comprehensive team of providers.
Our patient care ranges from pediatric to geriatric, ensuring that an entire family can be a part of our welcoming and collaborative atmosphere. As a long-standing practice, many of our patients are on a first-name basis with our staff, allowing for well-established relationships between our patients and team members.
As a family practice we pride ourselves on our ability to offer the services of a larger primary care facility with the familiarity of a smaller family practice. Our primary focus as family physicians is preventive medicine that includes working to promote overall physical and mental well-being while also educating our patients on disease prevention and the management of chronic illnesses.
Our office utilizes Medent for the purposes of chart management, medical record retention, and internal communication. Experience with Medent is preferred, but training for operating within Medent will be available if necessary.
Requirements for the Position:
• A BE/BC in Family Medicine
• A NYS license
• A DEA
• Accreditation of Residency in the US
• Completion of Residency from Accredited Facility
Core Clinical Functions:
• Providing full-time direct patient care as a member of an expanded healthcare team.
• Providing outpatient primary care for a panel of patients, including routine and urgent visits.
• Prescribing medications within scope of practice.
• Ordering and reviewing diagnostic studies including laboratory tests and other diagnostic screening.
• Timely and effective documentation of patient notes in Medent.
• Reviewing health needs, ordering health promotion and disease prevention measures.
• Providing patients and their families with education regarding preventive care, chronic conditions, and treatments.
• Participating in team communications.
• Completing patient forms and responding to internal communication in a timely manner.
• Monitoring and managing an assigned patient panel for key population health metrics.
Additional Responsibilities:
• Supervising and/or collaborating with primary care mid-level providers, including clinical oversight and peer review.
• Supporting absent providers with handling urgent triages and secure messages.
• Occasionally supervising, educating, and evaluating medical residents and/or students.
• Attending and participating in internal staff meetings.
Compensation & Benefit:
• $160,000 - $220,000* annually

Clinical Nurse Manager

As the Clinical Nuse Manager, you will be the heartbeat of our clinical operations. You will bridge the gap between administrative goals and direct patient care. You will lead, mentor, and inspire our clinical staff, oversee daily clinical workflows, maintain strict regulatory compliance, and champion quality improvement initiatives.
- Recruit, onboard, train, and supervise all clinical support staff (RNs, LPNs, and MAs).
- Manage clinical staffing schedules to ensure optimal coverage, smooth patient flow, and minimized wait times.
- Foster a collaborative, positive, and accountable team environment focused on mutual respect and continuous learning.
- Oversee triage protocols, patient intake, rooming standards, and clinical documentation.
- Manage the procurement of clinical supplies, vaccines, and medications, ensuring proper storage (e.g., cold chain management) and equipment maintenance.
- Oversee infection control protocols, patient safety initiatives, and the proper handling of clinical incidents or grievances.
We’re looking for someone with:
- Valid NYS Registered Nurse (RN) license
- Minimum of 3–5 years of clinical experience, with at least 2 years in a supervisory or management role—ideally within primary care, family medicine, or an outpatient clinic setting.
- Strong proficiency with Electronic Health Records (EHR) systems (Medent) and Microsoft Office Suite.
- Experience with value-based care models, PCMH recognition, or population health management.
- Exceptional interpersonal skills with the ability to communicate effectively with patients, providers, and administrative staff alike.
- Ability to proactively identify bottlenecks and implementing creative, efficient solutions.
- Ability to stay calm, focused, and organized in a fast-paced, evolving healthcare environment.
Pay Range $80k-$87k per year depending on experience and we offer full-time employees' insurance and other benefits.

Clinical Nurse Manager - Apply
Registered Nurse (RN)
Working in our primary care practice offers a set schedule without having to work major holidays, overnights or weekends! We’re a supportive family practice looking to fill a full-time RN position at our Hamburg office.
We are looking for a NY licensed Registered Nurse to use clinical judgment and interpersonal skills to drive positive patient outcomes. This role requires an RN who can strategically identify and implement solutions to patient issues, working collaboratively and supportively to ensure exceptional, individualized care.
Key Responsibilities Include but are not limited to:
- Triage: Managing the triage flow and performing initial patient assessment, continuously collecting health data regarding the health status of patients.
- Direct Care: Administer provider-prescribed medications, treatments, immunizations, and injections.
- Operational Support: Facilitate efficient patient flow to maximize provider productivity.
- Care Coordination: Collaborate with staff to develop and implement customized, patient-centered care plans.
- Health Maximization: Partner with patients to optimize their health capabilities and well-being.
All of our nurses work Monday-Friday day shifts with one evening shift (1pm-9pm) per week.
